Freedom Functional Wellness is an out-of-network Provider and does not accept insurance. Our office cannot assist you with claim resolution. In addition, Lou Higuera, NP is not a Medicare provider. If requested, we can provide you with an itemized receipt (except for Medicare patients) that you can submit to your insurance carrier. You will not be able to submit any bill to Medicare for reimbursement for any services you encounter in our office. Lou does not submit her medical notes to insurance companies. Note that many health insurance policies do not cover functional medicine. Medical insurance has a powerful influence over what doctors can and cannot do for their patients. Having to follow insurance guidelines often limits treatment and causes patients to receive substandard care.
No. Freedom Functional Wellness is not a Primary Care Provider (PCP).
All services, including labs and mail-away specialty lab tests are self-pay. The office does not file insurance for lab tests. A substantial discount is given on labs ordered through Labcorp.
Lou does not prescribe Adderall, Vyvanse, any medication for ADHD, anti-depressant, Xanax, Ativan, Klonopin, or any benzodiazepine or controlled substance other than testosterone. There is a standard $25.00 service fee charged for pursuing prior authorizations from insurance companies for medications.
To streamline our billing process and ensure timely payment for services, all patients are required to keep an active credit card on file with our office. Charges are communicated in advance, and/or outlined in our Fee for Service Agreement.
Cash, check, and credit card (MasterCard, Visa, Discover) are all accepted methods of payment for services. We do accept HSA and FSA cards. When you schedule the initial visit, we request a credit card on file to hold the appointment for you. No charges will be applied to your credit card unless you miss or cancel an appointment without proper notice. On the day of your scheduled appointment, all charges for consultations, laboratory testing and nutritional supplements will be itemized and payment will be due. Over-the-phone or in-person consultations will be billed to your credit card on file unless you provide other payment information and instructions prior to your appointment. If additional lab tests are required and our office sends test kits, the appropriate fees will be charged to your account.
Lou Higuera, NP does not fill out medical disability forms for patients, nor does she submit her medical notes to support disability claims.
Phone messages will be responded to as soon as possible. To reach the office, please call 615-348-5098 or send a portal message if not urgent. If you call after hours (after 5:00 PM or on a weekend), please leave a message and the office staff will return your call as soon as possible. Please note the office is closed Wednesdays and Fridays. We are not an urgent-care clinic and do not have after-hours services.
Please DO NOT wear perfume/cologne to our office. Many patients as well as the provider are chemically sensitive.
There is a 48-hour (2 business days) cancellation and rescheduling policy. Your appointment must be cancelled or rescheduled at least 48 hours (2 business days) prior to your consultation time or you will be charged a $150 cancellation fee. There will be a 100% office visit charge for “no-shows”. You may cancel your appointment by calling the office at 615-348-5098 or emailing office@freedomfunctionalwellness.com.
